Types of Plans Available

Traditional Fee-For-Service
You go to the doctor and hospital of your choice. You may even select your own specialist without receiving a referral from your primary physician. The insured must usually meet a deductible (amount payable before company will reimburse any charges) and then typically pay for 20 percent of the claim cost. Insurance will pay for the reasonable and customary cost. The balance is your responsibility. This plan is more flexible and comes with a higher price tag.

 

Managed Care
This type of plan contains a “network” of physicians contracted by the company. If you go to a physician within the “network,” your out-of-pocket expenses would be less (usually low co-payment for office visit or service). You still have the option to choose a doctor outside the “network,” but you would pay more to do so.

 

Preventive Care Plans

These health plans are similar to traditional or managed care programs yet offer monetary incentives for participating in certain health risk assessments (smoking cessation, weight management program participation, etc.).

Health Savings Plans
Health savings plans are another type of preventive care plan where an insured sets up a tax-favored account to fund medical expenses with pre-tax contributions.
